Currently, we are in the process of converting our entire practice from paper charts to an electronic health record. In doing so, we have new forms that will need to be completed. We realize that this increase in paperwork may be an inconvenience. Please accept our sincerest apologies.
We are taking steps to make the conversion process as seamless as possible, maintaining excellent patient care and service, while learning a new process. We ask that you complete the necessary paperwork prior to arriving for your appointment. Current patients, the forms we are asking for you to complete are electronic health record forms. New patients, please use patient registration forms. If you do not have a computer or printer, please call our office and we will be more than happy to mail or fax the necessary paper work to you.
The sooner we receive your paperwork, the quicker it will be to register you in the new system. Please submit the paperwork by mail or fax to 214-265-1806. If it is more convenient you may bring the completed paperwork, with you, to your appointment.
Please arrive at least 20 minutes early and be sure to bring your driver’s license and insurance card.We look forward to seeing you and appreciate your patience and cooperation during this transition.
